flawed coins

I've read a bit about coins that left the mint, flawed. Does this still happen?

Yes, sometimes by accident and sometimes on purpose.
I believe it has gotten more difficult to make mistakes these days due to newer technology, but accidents still do happen from time to time.
Accidents such as double strikes, off center punches and the EXTREMELY rare two headed or two tailed coin. Not the magic store fake, but the legitimate, documented real thing, of which I have only heard of there being one of.
